Please use this link to take a content based quiz and complete the following steps:

1.     You will need to pass the quiz with a score of 80% or higher.

2.     You will complete survey information about the event and make sure to select Yes for “Would you like to receive continuing education hours/certificate of completion for attending/watching this webinar?”

3.     You will be asked to provide the name as you wish for it to be listed on the certificate and the email you wish for it to be sent.

 After passing the quiz, and completing these steps,  the certificate will generate and be emailed to the address you provided, it will come from no-reply@qualtrics.com so make sure to check spam or other email folders.
